The most comprehensive consultative service offered by FHSI is our outreach program assessment, which provides clients with a detailed and objective snapshot of their outreach program’s infrastructure and service delivery system. FHSI program assessments involve active participation from our clients and can be conducted either onsite or offsite.
With FHSI’s assessment reports, farmworker-serving organizations can:
Identify program strengths and areas needing improvement;
Improve or develop an outreach program plan that includes measurable goals, objectives, and action steps;
Use information from the report for grant applications; and
Prioritize program areas for FHSI consultation services or other program support needs.
Methodology:
FHSI’s assessment services focus on the farmworker health outreach programs infrastructure and service delivery mechanisms. To assess farmworker health outreach programs and related services, FHSI uses the following as a reference:
Our Integrated Community Health Outreach Model;
Our National Farmworker Health Outreach Guidelines; and
The goals and objectives outlined by the client organization for the outreach program.
Program assessments include:
A desk review of client materials, which may include marketing materials, web site, job descriptions, grants, organizational charts, Uniform Data System reports, migrant health plans, community health care plan and other relevant materials;
Key informant interviews with program staff, clinical providers, and administration; and
Shadowing outreach workers while they deliver heath education, case management, and other services to farmworkers (on-site assessment only).
Program Assessment Report:
After completing a program assessment, FHSI presents clients with a Program Assessment Report that features:
Summary of client’s services, staff, farmworker demographics, and community resources;
Identified programmatic areas of strength and need; and
Recommendations and strategies that address areas of improvement within the organization’s program infrastructure and service delivery systems.
